GP Professional Development Grant Application

Please complete and submit the form below to apply for a WellSouth General Practitioner Professional Development grant.

Please note, to be eligible you must:

• be vocationally registered General Practitioners participating in the RNZCGP Te Whanake CPD programme
• be normally residing in Otago or Southland
• be currently working in a general practice in the WellSouth network, and
• have worked on a regular basis for a general practice in the WellSouth network for at least six months prior to application
