You can apply for a grant of up to $4,000 (or up to $5,000 for applicants working in a rural practice) to support the activities selected above (the form will set this limit automatically based on the Practice you’ve selected above) Please note, If your application is audited, you will be required to provide proof of purchase support your claim.
